The forms listed below are required for each Swimmer/Diver for each Swimming and Diving season.
All forms, including a physical, must be completed each year and on file prior to participation in any practice sessions.

Information Sheet - Fill out (on your computer), print, sign and return to your coach.

FISD Swimming & Diving Policy Packet - You will be able to sign for each of the forms in this packet on the Information Sheet. These forms will remain on the web site for the entire swimming and diving season.
The following forms may not be required of all Swimmers/Divers. They are available if they are needed.

Previous Participation (Transferring from another School or School District) Please contact and meet with your coach to get and fill out the necessary forms.
