Student Immunization Records

The Frisco ISD - Student Immunization Shot Record report displays the shot records that student guardians have provided to their child’s campus. If the shot record appears inaccurate or out of date, please contact your student’s campus nurse.

How To Access Immunization Records

• Click here to access the report and log in using your Portal username and password.

• If you forgot your username or password, access  The Portal information page for assistance.

• Find and click on the tile called Immunization Records.

• Select the student you will run the Immunization Shot Record for, then click on the Run Report button at the bottom.

• If the Run Report button is grayed out, please be sure you have selected one student.

• Hover over the right corner to view the download icon and download the document if you need a copy to print or save. To choose another student, return to the report page to select another student.
