• American With Disabilities Act (ADA) Public Place Request Form - Google Form
  This form is intended only for individuals requesting accommodations to access District buildings and programs as public accommodations under Title III of the Americans with Disabilities Act (ADA).
  
  Requests related to student accommodations should not be submitted through this form and must instead be directed to the District’s Section 504 Coordinator/ADA Coordinator in accordance with District policy.
  
  Upon submission, you will be contacted within two (2) business days to confirm receipt and request any additional information that is needed to process the request. Please be advised that while Frisco ISD will make every reasonable effort to accommodate requests, we cannot guarantee that accommodations will be available in all circumstances.
