All upcoming 7th Graders MUST HAVE A PHYSICAL in order to participate for the 2009-2010 School Year. Please make a copy for your records.
All returning 8th Graders...Fill out a new Medical History form and turn it in...if there has been a major illness or medical condition, a NEW Physical must be submitted for the 2009-2010 school year.
