This form is ONLY for classes offered at the Career & Technical
Education Center (CTEC). Most of these courses require teacher approval. Students
are responsible for ANY & ALL prerequisites in their program of study. All
students must fill out this form completely. If your program requires an additional application, please print and complete the application then return it to your counselor.

We, the student members of an academy within the CTEC, agree:

Commitment
We understand acceptance in one of the academies in the Frisco ISD Career
& Technical Center involves commitment from parents and students.
- Students must follow student handbook guidelines established by the CTEC campus
and the Frisco ISD High School Code of Conduct and any applicable handbooks or FISD
policies and procedures.
- Repeated disciplinary offenses, safety violations, or placement at the SOC could
result in removal from the CTEC.
- Students must respect all individuals and property belonging to fellow students,
teachers, and the CTEC.
- Each student will be expected to participate in a student organization through one
of the academies.
- Parents are asked to attend program functions and meetings.
- Parents & students must accept the district’s transportation contract.
- Transportation for FISD students is provided from the student’s FISD home high school
campus to the CTEC and back if needed during the school day.
- Students are also allowed to make their own arrangements for transportation for
them selves only and must arrive to and from their home campus before the applicable
tardy bell.
- Transportation is not provided for students residing outside FISD boundaries.
