PTA/PTO/Booster Club Insurance Instructions: View Information
Certain events may require proof of insurance coverage either by Frisco ISD or the vendor providing services. Please see the attached information sheet regarding certificates of insurance or proof of "self-insurance" coverage, please send written requests to the Risk Management Department, including a copy of the original document (contract, letter) requesting the information. The written request should provide basic information as to the activity, services, or event to be covered, including the dates and to whom the certificate should be issued.

Frisco ISD Student Accident Insurance Program 
(Policy Year: 8-1-11 Through 7-31-12)
Parents and Guardians,
We are pleased to announce that GM-Southwest, Inc. will continue to be the student insurance administrator for our 2011-2012 school year.
The Frisco Independent School District provides an accident plan to 7-12 grade students participating in UIL/Athletic activities. Accidents occurring during scheduled practice sessions, games, tournaments, events at other schools, and/or travel to and from activities are covered under this plan.
In addition, all student grades PK-12 are enrolled in a School Time Accident Policy that covers all non-athletic accidents occurring on school ground during normal school days, including school sponsored and supervised extracurricular and club activities.
In addition to the supplemental accident plans provided by Frisco ISD, GM-Southwest is also offering the following product options to Frisco ISD parents & guardians:
