Gather your information before starting this process

We suggest that you send a copy of each page that you fill-out to the printer if you would like to have a personal record.

Make sure that all required information is entered. If you do not enter all the required information on the first page the program will not advance to the next section.

If you have a slower connection via the phone line please allow a minute or two for the transactions to take place. Example: it typically takes a minute or so to retrieve the long list of colleges that you will select from.
