Enrollment & Registration - 2009-2010 School Year

Below are forms that are used during enrollment and registration. Contact your school for specific dates and times for registration and enrollment. For zoning information visit www.friscoisd.org/zones.

Forms that can only be filled out at the time of registration or enrollment are not posted here. These forms are intended to be filled out at the campus:

• Registration/Student Information Form
• Home Language Survey
• Student Residency Questionnaire
• Migrant Education Program Employment Survey

Please note that some information has to be updated annually.

First time enrollment:

• Enrollment Checklist: English | Spanish
• Information of Enrolling Parent or Guardian: English | Spanish
• Kindergarten Placement Form: English | Spanish

Please provide the following at the time of enrollment:

1. Student Birth Certificate (or passport) - School will copy
2. Student Social Security Card - School will copy
3. Immunization Records
4. Proof of Residence (Current utility bill, lease agreement, contract of sale or closing papers)
5. Drivers License of person completing enrollment (Per Senate Bill 1432)
6. Last Report Card/Unofficial transcript
